All pharmacists and pharmacy students who work within Sona are required to read the following statement regarding patient confidentiality.

 

All information pertaining to patients who utilize Sona shall be held in the strictest confidence. There shall be no disclosure of any patient information to anyone outside Sona except as authorized by a Sona pharmacist. Discussion with other Sona Pharmacy staff should take place only when the information is necessary for providing health care services to the patient.

 

Patient records shall not be copied or removed from Sona Pharmacy except as authorized by the pharmacist in charge. A patient shall be permitted to review and have copies of his own records; however, for security reasons such copying of records shall be only upon the specific authorization of a Sona pharmacist. Release of such information to relatives of patients or to other health care providers and services may be made only upon a signed release, signed by the patient or by his guardian.

 

Patients expect and have the right to receive professional and confidential health care services. Every effort should be made to protect the patient's right to privacy.
